Helping you to get the right care, at the right time, is so important to us.

Our dedicated Outreach Nurse can visit you in the comfort of your own home to offer personalised care and support.

We can chat with you to help you with the practical decisions that need to be made and support you to take in any information you may have been given by your GP, and answer any questions you may have. Our Outreach Nurse also works very closely with our Occupational Therapist, Social Worker and Physiotherapist and can do joint visits when necessary.

And, if it’s the right timing and decision for you, we can refer you to our hospice and help you to access our care. Whether that’s coming along to our Wellbeing Centre or staying on our Inpatient Unit, we can help you to make choices about your care.

We support not only you, but those closest to you too. Offering support and guidance for your loved ones who may have questions themselves.

Once you have started to access our care, our Outreach Nurse can continue to support you if needed, with home visits and support after you have been discharged home. 

You can be referred to our Outreach Nurse through your GP, palliative care specialist nurse (including Macmillan and District Nurses), a doctor or consultant at a hospital, or other health care professional.

Referrals to our Outreach Nurse are often made at an early stage.

We always do our best to contact you within a week of receiving your referral.

Once you have been referred, your Outreach Nurse will phone you to have a chat about your needs. Sometimes advice over the phone may be all that you require but in most cases your nurse will work with you to arrange a home visit.

At your first home visit, your Outreach Nurse will make a full assessment of your care needs and plan any ongoing support you may need in the future.

If you need a few home visits first before we help you to access our care, that's fine. We can tailor our support to your needs.
Our Outreach Nurse can offer some types of care, including administering medication, however, we don’t provide the generalist nursing care that you might get from your District Nurse or care agency. We do provide specialist care assessments and specific interventions which help relieve symptoms and avoid delays in essential care. We keep in close contact with the healthcare professionals involved in your care, e.g. GP or District Nurse.
Yes. Our Outreach Nurse can talk to you about any concerns you may have about accessing hospice care. Your nurse will work really closely with you to work out what's best for your needs. Your nurse can also help to sign post you to other support services, or other members of our team such as our Occupational Therapist, Social Worker or Physiotherapist who can help too.
Yes. If you are feeling worried or have any questions about our Inpatient Unit, our Outreach Nurse can help to answer your questions and talk to you about the information you have been given.
