After your referral, you will have an initial assessment with one of our nurses, either on the telephone or via a video link, and we’ll talk about how we can help you. If you require one to one support, you will be allocated a named nurse.

To be a part of the virtual sessions, both groups & one to one, you will need access to Zoom on a computer, tablet or smart phone.

Top tips for Zoom

Most people access our services for a limited time, until they are ready to be discharged. This will assessed by your named nurse and the multi-disciplinary team, and if needed we will support you with your discharge. Any ongoing specialist care support may be at our hospice, or with the Community Macmillan team.
